SAPHOとはA or P or H or Oなのですね。

Synovitis, Acne,Pustulosis,,, これを上の空で言えたら、かっこいいですよね。

SAPHO症候群の最大の観察研究は、120名からなるフランスからの論文（1990年）です。

この症候群の基準は、

１．重症痤瘡の骨関節症状

２．掌蹠膿疱症の骨関節症状

３．過骨症（皮疹の有無を問わない）

４．慢性再発性多巣性骨髄炎（皮疹の有無を問わない）

のいずれかとされています。感染症やDISHなどを否定できれば診断してよいと。

すなわち、SAPHOとはA or P or H or Oなのです。

結構、甘い。。

また、SAPHO症候群120例のうち、37例は尋常性乾癬を有していたとのこと。。

最近なら、乾癬性関節炎と分類されやすいでしょう。

著者は考察で、SAPHOと乾癬性関節炎の違いについて、SAPHOは骨の病変がはっきりしていることを挙げ、両者を分類するための基準を作るために研究中であると述べています。

この研究はいまだ報告されていません。

それにしても、覚えなければいけないような複雑な名前を付けるのではなく、

痤瘡性関節症、掌蹠膿疱症性関節症、特発性過骨症等のように、よりSpecificな病名にならないものでしょうか。

ひょっとすると、SAPHOを覚えなくてもいい時代がくるかもしれませんよ。

A. Inclusion features

1. Osteoarticular manifestations of severe acne

2. Osteoarticular manifestations of palmoplantar pustulosis

3. Hyperosteosis with or without dermatosis

4. Chronic recurrent multifocal osteomyelitis involving axial or peripheral skelton, with or without dermatosis

One of these 4 items is sufficient to diagnose the syndrome, in the absence of an exclusion feature.

Features of ankylosing spondylitis

Presence of low virlence bacterial infections

C. Exclusion features

Septic osteomyelitis
